Required Documents for Locum Contract Submission – Step-by-Step Guide

If you have been selected for a locum position in Kuwait, it is important to prepare and submit the following documents in the correct order. These documents are mandatory for the processing and finalization of your Locum Contract:

Required Documents:

• Entry Visa Copy (with Kuwait entry stamp)

• Passport Copy
• Passport Page showing the Kuwait Entry Stamp

• Attested Medical Qualifications (Degree/Diploma) – attested by Kuwait Embassy
• Attested Experience Certificates – attested by Kuwait Embassy
• Medical License/Registration Certificate
• Medical Fitness Certificate
• Police Clearance Certificate – attested by Kuwait Embassy

• Updated Curriculum Vitae (C.V)

Important: Ensure that all copies are arranged in the above order and attached to your signed Locum Contract.

Locum Contract Instructions:

• Sign on the left-hand side of the Locum Contract.
• Return all 3 pages of the signed contract along with the documents listed above.
• Carefully verify the following details in your Locum Contract:
  • Date of Joining
  • Full Name (exactly as in your passport)
  • Passport Number
  • Locum Salary

If there are any errors or mismatches in these details, do not sign the contract. Instead, notify us immediately so we can correct and reissue the updated contract for your signature.
